An individual care plan is a summary of the needs and service options developed by the client and their aged care support provider to meet the client's needs. It can include what, if anything, the person can do alone, with little assistance, or if they need total care. It contains details about medical, physical, social, emotional, lifestyle and spiritual care needs and outlines how the person wishes services to be delivered.This course helps care workers understand the importance of individual care plans and their duties when it comes to implementing them.
